The 5 C’s of palliative care.

Commitment: First and foremost is the commitment to deliver compassionate care for patients and their families.
Compassion: Human dignity always comes first in compassionate care. Compassionate Care is at the very heart of what we do and why we exist. It is doing the right thing for the right reason.
Competence: Competence is to provide the right care at the right time and at the place of your choice. Be it at home, hospice or at an acute care setting.
Communication: Helps in discussing values and goals of care with patients and families. Helping patients understand choices and consequences. Translating values and goals into concrete care plans.
Community: Community support plays a big role in providing Compassionate Care. The collaborative support of volunteers and other community entities helps in identifying and providing palliative care to needy patients.
